DialysisCare+: A Mobile Personal Health & Treatment Tracking System for Dialysis Patients

About the Project

DialysisCare+, a comprehensive mobile health application designed to assist Chronic Kidney Disease (CKD) patients in managing their dialysis treatments and daily lifestyle protocols. Currently, patients rely on error-prone manual paper logbooks to track crucial metrics like fluid intake, weight, and blood pressure, which lack real-time clinical feedback. DialysisCare+ replaces these manual logs with an automated digital ecosystem featuring real-time fluid balance calculation, predictive AI health alerts, Third-Party EMR API integration for lab results, and a real-time clinic scheduling system. Additionally, the app introduces a Virtual E-Medical ID for emergencies and a Medical Sponsorship Gateway for financial aid. By centralizing self-care, clinical administration, and financial welfare into a single platform, this project aims to reduce medical complications, lessen the burden on caregivers, and empower patients to manage their chronic condition safely and efficiently.
